A new study by the Food and Drug Administration and the National Institute of Health shows that some drugs can make the difference between prescription and over-the-counter (OTC) use.
“We are very concerned about these drugs because they are often used to treat erectile dysfunction,” said Dr. Steven Nissen, the director of the Center for Drug Evaluation and Research at Brigham and Women’s Hospital. “There is no study that has looked at this.”
The National Institutes of Health estimates that about one million American men suffer from erectile dysfunction, and nearly a third will go into treatment. If these drugs are effective, about one in five will be prescribed OTC.
“We are concerned about these drugs because they are often used to treat erectile dysfunction,” Nissen said.
The study was part of the FDA-sponsored, “Drugs in Specialty” clinical study. This was designed to examine whether the drug sildenafil, marketed as Viagra, could improve blood flow to the penis.
Sildenafil is a phosphodiesterase 5 (PDE5) inhibitor used for the treatment of erectile dysfunction. It is used to treat pulmonary arterial hypertension, a condition in which a blocked blood flow to the lungs causes shortness of breath and rapid breathing. It works by relaxing the walls of the blood vessels so that the blood can flow more easily.
Sildenafil has a favorable side-effect profile, with an average onset of action of 5.5 hours, versus 6.5 hours in the placebo group. This is the first study to examine the effects of sildenafil on blood flow to the penis in patients with erectile dysfunction.
A study of patients taking sildenafil for up to six months found a statistically significant improvement in erectile function (i.e., improved self-assessed ability to achieve an erection) compared to a group that did not take sildenafil. Patients who took the drug also showed improvements in self-assessed ability to achieve an erection and in the duration of sexual activity.
Nissen said, “This is a very important finding.”

In a separate study, Nissen and colleagues examined whether Sildenafil use could be beneficial for men with ED.
“It’s important to note that while this study is a small, small study, this study should be replicated with a larger, more consistent sample size,” Nissen said. “We do not know if the effects of Sildenafil on ED patients are permanent, long-term, or that it may be used for other purposes.”
If so, the study would be published online in the June issue of theJournal of the American Medical Association.
In the study, Nissen and colleagues compared blood pressure before and after two standard treatments (Viagra and sildenafil) in men with ED. They found that the two treatments improved erectile function, compared to the placebo group. Sildenafil improved erectile function in a small, placebo-controlled study, and the drug did not improve erectile function in men with ED.
The researchers also compared the two treatments in a randomized, double-blind, crossover study. After two treatments, they found that sildenafil improved erectile function in only about one percent of men in both groups, while sildenafil improved erectile function in about one-third of men in both groups. The researchers found that the two drugs improved the duration of sexual activity by about one-fifth of men in the sildenafil group and about one-quarter of men in the placebo group.
Nissen and colleagues concluded that sildenafil is safe and effective in men with ED.
